Position Overview:
The Quality Coordinator works in collaboration with the Director of Quality and other Ambulatory Medicine leadership to promote processes to improve clinical outcomes, quality metrics and prepare for regulatory/accreditation surveys. As needed identifies best practices, collaborate with others to develop standard workflow/processes to promote the highest quality of care to achieve the best outcomes. Under the direction of the Director of Quality, this role develops and maintains internal key relationships with Ambulatory Management leadership, Medical Directors front line staff and others across the Community Memorial Healthcare organization. Externally this role builds, maintains relationship, and is a key contact with payers, Accountable Care Organization (ACO) and other vendors. Participates on key committees to represent and lead quality and performance improvement activities. The Quality Coordinator facilitates internal quality reporting and is the liaison to external reporting, such as health plan, ACO and other outside entities.
Quality Coordinator is the internal resource for quality reporting, quality processes, HEDIS and Health plan quality metrics and quality incentive programs such as HCC, STAR, P4P, NCQA and ACO. This role has expertise of extracting and analyzing quality data from the electronic medical record. The Quality Coordinator communicates and facilitates quality outreach to patients.

This position performs resident/patient care activities and related services necessary in caring for the personal needs, safety, and comfort of residents/patients as assigned. Assists in providing a positive physical, social, and psychological environment. The CNA II relates positively, effectively, and appropriately with residents/patients, families, community members, volunteers and other facility staff. Successful incumbents have fast and adaptive problem-solving skills who value collaboration and teamwork. This position has an in-depth understanding of patient care policies and regulations and is comfortable working with specific populations of patients, including elderly patients and those with special needs.
Essential Functions and Responsibilities:
1. Assists with resident/patient care needs and comforts including but not limited to:
- Assists or gives oral hygiene and mouth care.
- Assists with bathing functions and dressing/undressing as necessary.
- Assists with nail care, shaving and hair care.
- Assists with lifting, turning, positioning and transporting residents.
- Makes both occupied and unoccupied bed (as assigned).
- Keeps residents/patients dry (i.e. change gown, clothing, linen, etc.) when they become soiled or wet.
- Assists resident/patient with bowel or bladder functions.
- Checks resident/patients frequently to assure that their personal care needs are being met.
2. Assists with nursing functions as follows:
- Measures and records vital signs.
- Weighs and measures residents/patients.
- Collects Specimens (i.e. urine, stool, sputum)
- Prepares and gives enemas.
- Provides daily indwelling catheter care.
- Performs diabetic urine testing.
- Turns bedfast residents/patients at least every two hours.
- Provides daily range of motion exercises.
- Assists with care of dying residents.
- Maintains accurate intake and output records.
- Noting observations of patient behavior, including complaints, or physical symptoms to nurses.
3. Performs food service functions including:
- Prepares resident/patients for meals (i.e. take to bathroom, wash hands, oral care, take to and from dining room, etc.).
- Serves food trays. Assists with feeding as indicated.
- Records the residents/patients food fluid intake.
- Serves between meal and bedtime snacks.
- Keeps resident’s/patient’s water pitchers clean and filled with fresh water on each shift.
- Performs after meal care (i.e. clean residents/patients hands, face, clothing, brush teeth, take to bathroom, etc.).
- Notes and reports changes in resident/patient eating habits, especially those on diabetic diet.
- Assists with keeping nursing and resident/patient care area neat, sanitary and orderly.
- Floats to the OVCH Acute Care Facility four (4) times each year.
- Appropriately records and charges for supplies.
- Performs other duties as assigned.

Prepare medications under the direction of a pharmacist with focus on Specialty Pharmacy. May measure, mix, count, label, and record amounts and dosages of medications according to prescription orders. Participates in stock management by ordering, receiving and shelving product. Processes and adjudicates prescriptions through the electronic system. Works with insurance companies to assure billing and reimbursement. Processes patient charges and billing. Dispenses completed prescriptions to clients. Maintains prescription records. Prepares medications to be shipped to patients. Finds and enrolls patients in copay assistance programs. Input patient data into Patient Management Program.
Position Overview:
Provides pharmaceutical services supporting all outpatient activities with focus on Specialty Pharmacy. This includes prescription processing, patient counseling, technician oversight, clerk oversight, record keeping, billing and adjudication (prior authorization), physician communication, computer operating system use and record keeping. Enroll patients in Patient Management Program to create personalized disease state and specialized drug care plans. The patient management program will help each patient manage side effects, promote adherence to medications, and improve overall health. Create and manage performance improvement and quality improvement plans for Specialty Pharmacy adherence. Performs all duties conforming to CMHS organizational standards and regulatory requirements.

Supervisory Responsibilities:

This position under the direction of the Information Systems Supervisor is responsible for ensuring the confidentiality, integrity, and availability of CMH data assets, wherever they may reside. The primary role of this position is to prevent and mitigate cyber-security incidents such as application penetration, malware and social engineering attacks. The Information Security Project Manager must have the ability to analyze, recommend, develop, implement, and document systems and processes in a manner which best protects CMH and its affiliated partners.
This position will identify potential gaps, review security incident reports, architect solutions and provide recommendations focused on enterprise security, response and remediation plans.

An essential daily job responsibility of an EVS Tech will be the cleaning of patient rooms for the hospital, including the possibility of cleaning COVID-19 patient rooms and/or possible contact with COVID-19.
As CMHS takes employee health and safety seriously, CMHS EVS Department will provide Personal Protective Equipment - PPE (mask, gloves, etc.) along with education and training for all EVS Techs to keep hospital employees and patients, as well as the community safe.
